All information is kept private and not shared with anyone outside of Rise Fitness coaching staff without prior permission from a parent or guardian. If the athlete has a behavioral plan, we require it be emailed to us BEFORE the athlete attends ANY classes.

Is there currently a behavior treatment plan in effect for the athlete? If you have a behavioral plan, it is a requirement that this plan be emailed to us BEFORE you attend your first class.
